Social Work Case Manager

Under supervision, you will perform psychosocial assessments, counseling, and discharge planning with minimal supervision. You must hold a master’s degree in social work with CSWE accreditation, NYS LCSW licensure or pending exam, and at least four years of post-LMSW experience in healthcare.
