Hospice Registered Nurse (RN) Case Manager – Full-Time | Evanston, IL JourneyCare Hospice We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Evanston area. Why Nurses Choose JourneyCare Over Other Hospice Agencies Only 1 weekend every 12 weeks (significantly less than many competitors) Focus on quality of care over visit volume Supportive leadership and interdisciplinary team Autonomy in the field without feeling isolated Schedule & Territory Monday–Friday, 8:30 AM – 5:00 PM One weekend rotation every 12 weeks and 1 summer and 1 winter holiday Territory: Evanston, IL and surrounding communities $80,000 – $97,000 annually (based on experience) 401(k) with company match Medical, dental, and vision insurance (effective first of the month after hire) Generous PTO and paid holidays Hospice certification support with salary increase Employee referral bonuses and discount programs Key Responsibilities Manage a dedicated hospice caseload as the primary RN Case Manager Perform patient assessments, admissions, and individualized care planning Deliver skilled nursing care focused on comfort and dignity Coordinate care with physicians and interdisciplinary team members Conduct routine visits and adjust care plans as needed Provide end-of-life support, including family education and pronouncement Supervise Home Health Aides in compliance with regulations Qualifications Graduate of an accredited RN program Current CPR and First Aid certification Hospice or home health experience preferred Strong clinical judgment and communication skills Valid driver’s license and proof of insurance #J-18808-Ljbffr
