As a valued member of our healthcare team, we are seeking a dedicated and compassionate LLicensed Practical Nurse (LPN) to join our team at Bayshore Nursing & Rehab in Glendale, Wisconsin. Our evening shift position offers a unique opportunity to make a meaningful difference in the lives of our patients and residents while working closely with our experienced and caring staff.
Responsibilities:
Provide direct patient care, including administering medications, wound care, and vital signs monitoring
Develop and implement individualized care plans to meet the unique needs of each patient
Collaborate with multidisciplinary teams to ensure comprehensive care and address any concerns or issues that may arise
Participate in daily reporting and documentation, including charting and record-keeping
Maintain a safe and clean environment, adhering to infection control protocols and ensuring compliance with policies and procedures
