Crisis Case Manager - Peer Aftercare Specialist Location: Central Road, Phoenix, AZ. Overview Terros Health, a healthcare organization dedicated to recovery, invites applications for a Crisis Case Manager - Peer Aftercare Specialist reporting to the Crisis Clinical Manager. This full‑time role supports patients and families in the transition from crisis care to ongoing recovery through counseling, resource coordination, and peer support. Responsibilities Provide direct interactions and service delivery to patients. Access and coordinate community‑based services to facilitate patient transition. Ensure linkage to community services and resources. Float between crisis department aftercare programs such as Bridging the Gap Crisis, Crisis Transition Navigator, and Community Stabilization Team as needed. Qualifications Peer Certification required; Certified Case Manager (CCM) certification highly preferred. Self‑identified, first‑hand experience with serious mental illness or a chronic disease (personal or family). Strong written and verbal communication skills. Flexible and collaborative, able to work autonomously and manage competing priorities in a fast‑moving environment. Experience with filing mental health petitions preferred. Commitment to each specialty’s goal of providing compassionate and empowering care to diverse populations, including chronic homelessness. Working Hours 40 hr week, Tuesday‑Friday 8 am‑6 pm (10‑hour shifts). Shift hours may vary if clinical or administrative needs arise.
