RN Home Health Weekend Premium

SHC WVUHS Home Care

Responsible for the direct, indirect, and case‑management nursing care of patients in their homes. The registered nurse collaborates with patients, families, and the interdisciplinary team (IDT) to develop, implement, and evaluate nursing care per agency and system policies. Minimum Qualifications Current Registered Nurse license issued by the state where services will be provided or a valid multi‑state RN license through the Nurse Licensure Compact (eNLC). Valid driver’s license. CPR certification within 30 days of hire. Preferred Qualifications One year of home‑care nursing experience. Two additional years of prior nursing experience. Core Duties and Responsibilities Perform functions of the home health aide. Execute and monitor basic patient care per policies and procedures. Provide a safe, comfortable environment in the patient’s home. Ensure privacy and respect the dignity of patients during all care activities. Complete assignments in a timely manner. Assume on‑call responsibilities per policy. Communicate patient care problems or potential problems to administrative staff and IB team members to maintain continuity of care. Provide care tailored to the patient’s age and needs (infant, pediatric, adolescent, adult, geriatric). Utilize admission criteria per agency guidelines. Document OASIS within federal guidelines. Perform full system assessments at admission and each subsequent visit. Apply appropriate nursing action in response to changes in patient condition. Report significant observed changes in patient condition to administrative staff and/or physician. Document patient status and services furnished per documentation policy. Document home health aide supervisory visits per standard. Perform CPR as directed by patient representative’s wishes. Use DME and medical supplies safely and maintain equipment. Demonstrate bag technique, apply and maintain equipment per policy. Apply cost‑effective supply usage. Maintain IV site integrity during patient care. Perform treatments and procedures per specific standards (vital signs, weights, specimen collection, ostomy care, catheterization, wound care, suctioning, gastric tube care). Teach patients per policy and procedures. Coordinate medication administration as ordered by physician and compliant with medication administration standards. Facilitate changes in level of care per the change‑in‑level standard. Recognize stages of death and dying. Complete plan of treatment, recertification, OASIS SOC/ROC, recertification, and discharge documentation per Medicare guidelines. Physical Requirements Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, and grasping necessary during nursing duties. Able to lift and move patients of all weights with appropriate assistance. Read and write legibly in English. Normal hearing and visual acuity. Occasional exertion of 20‑50 pounds, frequent 10‑25 pounds, or continuous up to 10 pounds to move objects. Must drive a motor vehicle safely. Working Environment Exposure to bodily fluids, human tissues, contagious diseases, sharps, and explosive gases. Exposure to toxic gases, fumes, and odors. Exposure to high stress and constant interruptions. Exposure to electrical current and radiation from X‑rays. Exposure to hazardous road conditions and weather extremes. Primarily performed in patients’ homes, often requiring extensive travel and unpredictable situations. Potential exposure to adverse or unpleasant home environments. Skills and Abilities Ability to perform complex mental activity in highly technical situations. Work successfully under highly stressful conditions, adapting to varying workloads continually. Make sound, independent judgments based on scientific principles and collaborate with a multidisciplinary team. Comprehend and execute oral and written instructions and procedures. Strong written and verbal communication skills. Math skills for drug dosage and infusion calculations. Willingness to work weekends, holidays, and all shifts. Knowledge of and compliance with safety policies and procedures. IV skills required. Additional Information Scheduled Weekly Hours: 36. Shift: Exempt/Non‑Exempt.
