Hospice Registered Nurse (RN) - Home Care

Job Description

We are seeking an experienced Hospice Registered Nurse (RN) to join a dedicated team providing compassionate home care. This role involves case management and delivering hospice care within a home setting. The ideal candidate will have a strong background in hospice services and the ability to support patients and families during challenging times.

Responsibilities

• Manage hospice patient care plans in collaboration with interdisciplinary teams.
• Provide skilled nursing services and emotional support to patients and families.
• Administer medications and treatments as prescribed.
• Conduct patient assessments and updates to care plans as necessary.
• Educate patients and families about end-of-life care options and procedures.

Required Experience / Certifications / Licensure

• Current and valid RN license.
• Experience with hospice case management.
• Previous experience providing hospice care in a home setting.
• Familiarity with McKesson/Netsmart EMR preferred.
• Experience with rural travel preferred.

Schedule

This position is scheduled for Monday to Friday, providing a consistent workweek.
