As a Hospice House Home Health Aide, you’ll be on an interdisciplinary team that delivers life-changing care to patients across Suffolk County.

YOU WILL

This role consists of two 12 hour shifts a week and some weekend coverage - Assist clients with personal care, including bathing, dressing, grooming, and mobility. - Provide companionship, emotional support, and encouragement in daily activities. - Monitor and promptly report changes in clients’ physical, mental, or emotional health. - Follow individualized care plans while ensuring clients’ safety and comfort at all times. - Assist with mobility, transfers, and positioning. - Support meals, feeding, and basic health-related tasks. - Maintain a clean, safe patient environment. - Communicate with nursing staff and report changes in condition.

QUALIFICATIONS

NYS Certified Home Health Aide. Ability to read and write English. Current NYS Driver's License. Ability to work well with others and accept direction. Flexibility.
