Key Responsibilities\n

• Facilitate access to vital community resources such as support groups, vocational rehabilitation services, and housing assistance, empowering patients to navigate their post-rehabilitation lives.

• Educate and support families on available community resources and post-rehabilitation options tailored to the needs of their loved ones.

• Actively participate in discharge planning processes, ensuring smooth transitions for patients moving to home care or other facilities, while maintaining continuity of psychological and emotional support.

You will primarily work with individuals receiving hospice care, which includes patients with terminal illnesses and their families. Qualifications\n

Compensation and Benefits\n
