This role will operate within a remote work setting, promoting flexibility while still ensuring impactful service delivery. As a part of a multi-disciplinary team at our hospice center, you will collaborate with healthcare professionals, parents, and caregivers to create tailored plans that support the health and development of children and families facing challenges. Key Responsibilities\n

• Conduct comprehensive evaluations and assessments for children, focusing on developmental delays and disabilities.

• Collaborate in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring alignment with family goals.
\n
• Monitor outcomes and progress as an integral member of the Early Intervention (EI) team.
\n
• Provide hands-on support to families, imparting valuable information and strategies centered around their child's development.
\n
• Engage with children and families to build rapport, fostering a healthy and nurturing learning environment.
\n
• Utilize evidence-based practices to recommend strategies in accordance with Natural Learning Environment Practices.
\n
• Implement IFSP services specific to the child's developmental status, taking into account family priorities and concerns.
\n
• Ensure compliance with all relevant federal and state regulations and standards dedicated to health and safety.

Patient Population / Client Focus\n

The primary focus of this role encompasses a vulnerable population of young children, particularly those with disabilities or developmental delays, and their families. By providing individualized support, you will play a key role in facilitating their access to essential services at our hospice center. Your efforts are vital in helping families navigate the complexities of early intervention, leading to better developmental outcomes for their children.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work.

• Current certification and licensure by the Department of Public Health.

• Active LCSW credential is mandatory.

Compensation and Benefits\n
