Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to identify the needs of children and families.

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSP) to ensure tailored support.

• Continuously monitor and evaluate outcomes as a collaborative team member, providing necessary adjustments to care strategies.

• Deliver educational support and resources to families to enhance their child’s development, focusing on physical, cognitive, social/emotional, and communication skills.
\n
• Utilize evidence-based practices and Natural Learning Environment Practices to design a tailored plan with each family.
\n
• Establish rapport with children and their families, facilitating a trusting relationship essential for effective support.
\n
• Provide ongoing coaching and guidance to families within natural settings to promote learning.
\n
• Ensure compliance with all relevant Federal and State regulations to maintain the integrity of services provided.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker with Willow Green, you will primarily work with children aged zero to three years who exhibit various disabilities or developmental delays. Our children require specialized attention to help them acquire essential skills necessary for their growth and independence. You will be instrumental in transforming challenges into achievements, fostering hope and growth for each child and their family.

• A Master's Degree or Doctorate in Social Work is required.
