Key Responsibilities\n

• Conduct comprehensive mental health assessments for patients, exploring their psychosocial history, current emotional state, and coping mechanisms, ensuring a tailored approach to their treatment.

• Develop and implement individualized treatment plans, collaborating with patients and their families to promote emotional well-being and facilitate the acceptance of their conditions.

• Deliver evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and supportive counseling, specifically designed for those facing terminal illnesses and their unique psychological needs.

• Provide individual and family therapy sessions to assist clients in navigating feelings of grief, loss, anxiety, and other emotional challenges related to their health status.

• Collaborate with a multidisciplinary team, including physicians, nurses, and chaplains, ensuring seamless communication and coordinated care for patients and their families.

• Offer crisis intervention services when needed, employing de-escalation techniques and making referrals for higher-level care if necessary.

• Educate patients and families about coping mechanisms and available mental health resources, promoting understanding and support.

• Maintain accurate and timely documentation of assessments, treatment plans, and progress notes in adherence to legal and ethical standards.

• Participate in routine team meetings and professional development opportunities, contributing to a positive, growth-oriented work environment.

• Stay informed about current trends and best practices in mental health care, particularly regarding end-of-life issues, to enhance patient outcomes.

Patient Population / Client Focus\n

The focus of this role is on individuals with terminal illnesses and their families, who require empathetic psychological support as they cope with significant life changes. Our patients often experience emotional distress, crisis situations, and complex reactions to their health conditions, necessitating a social worker adept in trauma-informed care and end-of-life counseling.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid LICSW licensure in Washington state.

• A minimum of two years of post-licensure experience in mental health or hospice settings.

• Strong understanding of grief and loss theories, end-of-life care practices, and evidence-based treatment methodologies.

• Proficiency in conducting mental health assessments and managing treatment plans tailored to hospice care.

• Ability to provide individual and family therapy, along with crisis intervention skills.

• Excellent communication and interpersonal skills, enabling the establishment of rapport and trust with patients and families from diverse backgrounds.

Compensation and Benefits\n
