Work Environment\n

This position is remote, offering you the flexibility to work from home while still engaging actively with clients through virtual means. You will primarily work within a hospice center's framework, which requires sensitivity and understanding toward the emotional and psychological needs of seniors and their loved ones. Regular video conferencing sessions will be scheduled, and face-to-face client interactions may occasionally occur in community settings or other agreed-upon locations.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, carefully evaluating their emotional, mental, and social factors that impact their overall well-being.

• Collaborate intimately with clients and their families to develop tailored care plans that resonate with their unique needs, preferences, and intended outcomes.

• Offer therapeutic counseling and emotional support, guiding clients through difficult life transitions, mental health challenges, and daily coping strategies.

• Act as an advocate for clients, ensuring they have access to necessary community resources and support services.

• Perform regular check-ins, monitoring client progress and needs through virtual consultations, while addressing any changes in their living conditions or mental health status.

• Facilitate effective communication among family members involved in a client's care, helping to resolve conflicts and build a supportive environment.

• Educate individuals and families about community resources, support groups, and programs to enhance social engagement and quality of life.

• Maintain accurate and up-to-date documentation of assessments, care plans, and all interactions according to ethical standards and regulatory compliance.

Patient Population / Client Focus\n

Your primary focus will be on seniors, particularly those engaged in hospice care or facing similar challenges. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• A current and valid state license as a Licensed Independent Clinical Social Worker (LICSW).

• At least two years of experience in a relevant social work role, ideally within community service settings focused on geriatric care.

Compensation and Benefits\n
