Key Responsibilities\n

• Conduct in-depth assessments of clients' psychosocial needs, considering various emotional, mental, and social factors, to tailor effective intervention strategies.

• Collaborate with clients and their families to develop personalized care plans that align with their specific recovery goals, ensuring a person-centered approach.

• Provide empathetic counseling and emotional support to clients navigating life transitions, implementing therapeutic techniques to promote resilience.

• Act as an advocate for clients, facilitating access to community resources and ensuring they receive the necessary support and services throughout their recovery process.

• Work closely with interdisciplinary teams comprised of nurses, caregivers, and healthcare professionals to coordinate care and address complex patient needs.

• Regularly monitor clients through scheduled virtual visits, assessing their well-being and intervening as necessary to address any concerns that may arise.

• Facilitate communication between clients and family members to enhance support networks and resolve any conflicts that may affect care.

• Educate clients and families about available resources, support groups, and programs that can significantly improve their social engagement and enhance quality of life during and after treatment.

• Maintain accurate documentation of client interactions, care plans, assessments, and progress notes using electronic health record (EHR) systems, ensuring compliance with ethical and legal standards.

• Stay updated with the latest practices and regulations in social work, participating in ongoing training and professional development opportunities.

Qualifications\n

• A Master's degree in Social Work from an accredited program.

• Current and valid state licensure as a Licensed Independent Clinical Social Worker (LICSW).

Compensation and Benefits\n

The salary for this position ranges from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
