Key Responsibilities\n

• Conduct comprehensive bio-psychosocial evaluations to identify barriers to treatment, enhancing patients' health status and facilitating resource utilization.

• Develop individualized treatment plans and implement ethical outpatient mental health therapies using evidence-based methodologies tailored to each patient’s needs.

• Consult and collaborate with medical and support staff to share insights on patients’ psychosocial functioning, ensuring integrated care plans that improve treatment adherence and coordination.

• Document social work interventions accurately in patients' medical records, following departmental standards to maintain up-to-date patient information.
\n
• Educate patients and their families on medical and social work recommendations, fostering understanding and compliance to assist in their journey towards optimal functioning.

• Advocate for patients’ needs through community resources, emphasizing self-determination and enhancing their social support networks.
\n
• Maintain professional expertise by engaging in ongoing educational activities, keeping abreast of current mental health practices and resources.

• Adhere to the National Association of Social Workers (NASW) Code of Ethics and departmental policies to ensure a safe and compliant environment for patient care.

• Analyze the effectiveness of treatment interventions, making necessary adjustments to maximize positive outcomes.

Patient Population / Client Focus\n

You will primarily work with patients who are experiencing varying degrees of mental health challenges, particularly those navigating the complexities of physical illness and psychosocial stressors. Your support will assist them in overcoming barriers that hinder their psychosocial functioning, allowing them to lead more fulfilling lives.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution and a current license to practice as a Licensed Clinical Social Worker (LCSW) in California.

• Proven ability to work with diverse patient populations, demonstrating an understanding of individual development stages and cultural considerations throughout the care process.

• Excellent analytical skills to evaluate patient needs and develop effective intervention strategies.

Compensation and Benefits\n

The salary for this position ranges from USD 85,000 to 100,000 annually, depending on experience and qualifications.
