Key Responsibilities\n

• Conduct in-depth clinical assessments to evaluate client needs and develop comprehensive treatment plans that address mental health concerns.

• Utilize evidence-based therapeutic techniques to deliver effective interventions tailored to each client’s unique circumstances.

• Maintain accurate and timely documentation of client progress, treatment plans, and outcomes in accordance with best practices and legal requirements.

• Collaborate closely with doctors, nurses, and other healthcare professionals within a multidisciplinary team, participating in regular meetings to ensure coordinated care and share insights about client developments.

• Actively engage in continuous education and training to remain updated on best practices in the field of hospice and mental health care.

• Support clients and families in navigating the emotional complexities of end-of-life issues, facilitating coping strategies and providing compassionate support.

Patient Population / Client Focus\n

This position primarily serves clients who are receiving hospice care, many of whom face complex emotional and psychological challenges intrinsic to end-of-life circumstances. The ideal LCSW will be adept at working with individuals who may be dealing with grief, anxiety, depression, or past trauma, providing a safe space for them to express their thoughts and feelings. The role will also require sensitivity to diverse backgrounds and needs, ensuring that care is equitable and tailored.

Qualifications\n

• A current and active license as a Clinical Social Worker (LCSW) in the state of Georgia.

Compensation and Benefits\n
