Key Responsibilities\n

• Conduct thorough bio-psychosocial assessments to understand the unique circumstances of each patient, identifying barriers to treatment and facilitating appropriate discharge planning.

• Develop individualized treatment plans based on evidence-based methodologies and clinical best practices, both for individual and group therapy sessions.

• Collaborate with a multidisciplinary team, providing insights into patients' psychosocial needs to enhance care coordination and treatment compliance.

• Regularly document social work activities in patients’ medical records adhering to departmental standards and conduct case-related statistical reporting.

• Provide education and resources to patients and families, fostering an understanding of mental health recommendations and promoting adherence to care plans.

• Advocate for patient and family needs within the community, enhancing access to social support services and resources.
\n
• Stay current with community resources and integration of services that may benefit patients and their families.

• Engage in continuous professional development through training and educational opportunities related to clinical responsibilities.

Patient Population / Client Focus\n

Qualifications\n

• Master’s Degree in Social Work from an accredited institution.

• Current LCSW licensure from the State Board of Social Work Licensure.

Compensation and Benefits\n

The salary for this position ranges from USD 85,000 to 100,000 annually, commensurate with experience and qualifications.
