Key Responsibilities\n

• Assume the role of therapist, working directly with clients diagnosed with significant mental illnesses and severe emotional issues, providing individualized care both virtually and in community settings.

• Actively engage in team meetings to strategize treatment plans and share insights that further the shared objectives of the organization.
\n
• Develop and implement evidence-based integrated care plans aimed at enhancing access to essential services, ensuring comprehensive care delivery for clients and their families.

• Conduct timely and appropriate screening assessments and arrange for thorough evaluations when necessary, while recognizing and mitigating risk factors that affect patient safety.

• Assist in crisis intervention efforts, providing immediate support to clients facing acute psychological threats, and intervene suitably to ensure stabilization.

• Monitor and ensure that services remain person-centered, effectively assessing the client’s needs throughout their treatment journey from initial admission to discharge.

• Contribute to ongoing quality improvement initiatives and participate in committees to enhance agency service delivery.

Patient Population / Client Focus\n

You will be working primarily with clients in a hospice setting, many of whom are experiencing profound emotional distress related to terminal illnesses. Your role will involve not only therapy but also the facilitation of family discussions and support to those involved in the clients’ care. Given the sensitive nature of this population, strong interpersonal skills, empathy, and a commitment to dignity and respect are essential.

Qualifications\n

• A Bachelor’s Degree in Social Work (BSW) and a current license as a Licensed Clinical Social Worker (LCSW) in New Jersey.
