Licensed Practical Nurse LPN

LPN Home HealthThe LPN position provides six patient visits per day. The role works within teams of RNs, PTs, OTs, and HHAs. This position requires home care experience. Under the direction of the Clinical Team Manager, the LPN is responsible for performing skilled nursing functions. This is in accordance with the State Nurse Practice Act and acceptable standards of practice, federal, state, and local laws. The LPN delivers care to patients as delegated by the RN Case Manager and/or Clinical Team Manager.
