Positional Overview

The Imagine Group is recruiting for a Patient Care Navigator on behalf of our client, a leading healthcare organization in Buffalo, New York , this provider delivers comprehensive medical services through a network of hospitals, specialty centers, and outpatient facilities. Dedicated to excellence in patient care, innovation, and community health, the organization offers employees the opportunity to make a meaningful impact while working in a collaborative and mission-driven environment.

In this role, you will be responsible for supporting patients and providers in Gastroenterology, Colorectal/General Surgery or Cardiology. This is a role in a centralized contact center, and the support is generally provided through inbound and outbound calling as well as digital communications.

Role & Responsibility:

Tasks That Will Lead to Your Success

Answer inbound calls from external providers, hospitals or patients who have referrals or new patient requests
Schedule patient consults and procedures
Document calls, clinical escalations and patient needs in the medical record
Process referrals from inbound faxes and work queues
Make outbound calls to patients for reminders, procedure coordination and engagement
Work collaboratively with Contact Center managers, Practice Managers, Supervisors and Team Leads to constantly improve service levels and workflows
Collaborate with the Centralized Referral Management team to coordinate patient care
Perform other duties as assigned by management

Skills & Experience

Qualifications That Will Help You Thrive

High School Diploma or Equivalent
2-3 years in a call center customer service environment, preferably in a healthcare environment
Specialty experience strongly preferred: Gastroentrology, Colorectal or Cardiology
Previous healthcare experience required
Basic medical terminology knowledge
EMR experience required, preferably with Epic
