Recovery-Focused Case Manager | Community-Based Support
Abraxas Youth & Family Services
RHD is seeking a dedicated Case Manager to support individuals on their path to recovery, stability, and independence. You will identify needs, coordinate services, advocate for access to resources, and help participants achieve personal goals. The role emphasizes community-based support, collaboration with partners, and ensuring documentation complies with regulatory requirements.
