The Birmingham VA Health Care is recruiting for a Physician - Patient Aligned Care Team (PACT) Primary Care Provider for Women's Health service. VA offers a comprehensive total rewards package. • Serves as a Patient Aligned Care Team (PACT) Primary Care Provider for a women's health panel and demonstrates advanced competencies and skills in assessing the physical and psychosocial status of patients.
• Maintains an active role in the assigned multidisciplinary health care team, promotes preventive health measures, and manages the acute and chronic conditions of assigned patients.
• Provides comprehensive assessment, diagnosis work-up, and clinic intervention. This includes performing comprehensive patient assessments, incorporating patient health history, performing physical examinations, and conducting patient education. Such care includes both scheduled and unscheduled appointments.
• Acts as the clinical lead of the assigned PACT team to coordinate and follow-up on patient care. Includes daily huddling, scrubbing of schedules, pre-visit assessments, review of tests, patient notification of test results, and development of follow-up care plans.

Work Schedule: Monday - Friday, 7:30 a.m. - 4:00 p.m. (subject to change based on the needs of the organization)
