Title
Physical Therapist - Physical Therapy

Requirements
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license in the state of practice.
- Experience in skilled nursing facility or geriatric rehabilitation preferred.
- Strong clinical, communication, and organizational skills.
- Ability to work both independently and as part of an interdisciplinary team.

Roles & Responsibilities
- Evaluate residents to assess mobility, balance, strength, gait, and functional limitations.
- Develop and implement individualized treatment plans to improve physical function, reduce pain, and prevent disability.
- Deliver direct therapy interventions, including therapeutic exercises, gait training, and balance retraining.
- Monitor residents' progress and adjust care plans as necessary to meet therapy goals.
- Participate in interdisciplinary care team meetings and assist with discharge planning.
- Educate residents, families, and caregivers on home exercise programs and safety strategies to encourage independence.
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure requirements.
- Document treatment sessions and progress notes accurately and in a timely manner.
