- Clean malpractice - [HIGHLY PREFERRED]
- Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED]* no exemption, required at presentation
- Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - [REQUIRED]
- DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) -- [REQUIRED] * no exemptions
- Malpractice coverage provided; CME allowance included
- Board Certified
- Annual salary range: $291,000 - $324,000 (full-time)
