Job Summary and Responsibilities

As our Licensed Vocational Nurse (LVN), you will provide vital direct patient care, significantly contributing to patient well-being and recovery as an integral part of our compassionate healthcare team.

Every day, you will assess patient conditions, administer medications, perform prescribed treatments, and meticulously document care. You will collaborate with RNs and physicians to implement individualized care plans, ensuring a seamless and high-quality patient experience.

• Provides direct patient care, evaluates outcomes, consults with other practitioners as required and adjusts nursing care processes as appropriate to ensure optimal patient care under direction from the RN.
• Assists physicians and other members of the healthcare team with the implementation of wound treatment plans.
• Functions as an intake/discharge nurse during clinic times which includes assessing, staging, measuring and photographing wounds accurately and in accordance to policy.
• Adapts planned educational programs and information to individual patients and family by modifying teaching strategies or content and integrates education during the delivery of patient care.
• Completes appropriate documentation which includes patient records ensuring it is completed according to hospital policy.
• Performs data entry into appropriate software as required which includes ensuring data is entered in a timely and accurate manner.

Job Requirements

Required

• One year recent outpatient experience in a medical environment
• Licensed Vocational Nurse: CA LVN
• Basic Life Support - BLS through an American Heart Association

Preferred

• Wound Care experience in a medical environment
