Since 1985, BHCHP’s mission has been to provide or assure access to the highest quality health care for all individuals and families experiencing homelessness in greater Boston. We care for over 11,000 homeless individuals each year and ensure that every one of these individuals can receive comprehensive health care, which includes primary care, behavioral health, medical respite, dental, case management, and more. Our staff work in 30+ locations across the city to serve some of our community's most vulnerable—and most resilient—citizens.

Community Care in Reach (CCiR) is a dynamic, multidisciplinary mobile addiction care team that brings compassionate, low-barrier medical and addiction services directly to people in Boston neighborhoods with high rates of overdose death. Through street-based outreach and a mobile medical clinic, the team engages people who are often disconnected from—or face significant barriers to—traditional health care and addiction treatment.

Are you looking for your next RN leadership role!

We are seeking an RN Manager of Mobile Addiction Services to play a central leadership role in this innovative program, helping the team continually adapt its model to reach people who might otherwise go without care. In partnership with medical leadership, you will help shape the strategic direction of CCiR, strengthen community partnerships, identify new outreach approaches and service locations, and improve the quality and accessibility of care.

In this role, you will also provide operational leadership for the mobile team and works alongside CCiR clinicians and case managers in the field, providing high-quality, equitable, patient-centered nursing care.

• Partner with medical leadership to develop and implement the strategic direction of CCIR, including new models of programming, outreach approaches, and service locations; build and maintain relationships with community stakeholders, neighborhood partners, funders, and collaborating organizations to strengthen access to mobile addiction care.
• Lead CCiR team meetings; establish agendas, facilitate discussion, and ensure timely communication of operational, clinical, and safety issues; oversee day-to-day mobile clinic operations, including staffing schedules, onboarding and orientation, DPH compliance, supply and equipment management, and coordination of vehicle maintenance and repairs.
• Serve as the primary operational liaison with internal BHCHP teams and key external partners, including AHOPE, the Kraft Center for Community Health, BPHC, and DPH/BSAS; represent BHCHP at statewide mobile addiction meetings and other relevant partner meetings.
• Promote team safety through proactive risk assessment, clear safety procedures, incident debriefing, and timely communication of safety concerns.
• Manage program administrative resources, including petty cash and patient incentive cards, in accordance with BHCHP policies.

Quality Improvement and Grant Management

• Lead population health management and clinical quality initiatives for the mobile program, including use of patient registries and team-based workflows to improve BHCHP quality measures.
• Develop and test practical approaches to improve preventive care and vaccination rates in the mobile setting.
• Draft program goals, workplans, outcomes, progress reports, and budget amendments as required by BSAS and other funders; monitor progress toward grant deliverables and participate in DPH site visits.

Clinical Responsibilities

• Provide nursing care as a member of the CCiR and Community Access teams approximately 2-4 shifts per week, based on program needs. Sites may include the CCiR mobile clinic, Finland Clinic, SPOT, and other outpatient sites as needed.
• Provide high-quality, equitable, patient-centered care to people with active substance use disorders and people experiencing unsheltered homelessness, including:

• Preventive care, including vaccinations and cancer screening when feasible.
• Urgent and episodic care, including wound care.
• Care relevant to people who use drugs, including PrEP/PEP and complex wound care.
• Screening for sexually transmitted infections and medication refills as appropriate.

• Maintain accurate, timely, and complete medical records in accordance with BHCHP standards.

Additional Responsibilities

• Drive the CCiR mobile unit when necessary and submit required driver's license and driving record documentation annually.
• Protect patient privacy and confidentiality and comply with HIPAA and BHCHP policies.
• Complete required BHCHP trainings, performance review activities, timekeeping responsibilities, and at least two equity-related education classes annually.

Qualifications

• Current Massachusetts Registered Nurse license required.
• At least two years of experience providing addiction-related services, with demonstrated commitment to patient-defined recovery and care that does not require abstinence.
• Exceptional organizational, communication, and team leadership skills, with the ability to manage multiple priorities in a dynamic outreach environment.
• Experience working with people experiencing homelessness, incarceration, sexual violence, and/or conducting street-level outreach with populations facing significant barriers to care strongly preferred.
• Commitment to equitable, culturally responsive, nonjudgmental care for diverse populations, including communities of color, LGBTQIA communities, non-English-speaking populations, people with substance use disorders or behavioral health challenges, and people experiencing homelessness.
• Bilingual Spanish/English preferred.
• Current life-support certification (e.g., CPR, BLS, or ACLS) from a course with a hands-on, in-person component, or ability to obtain certification within 60 days of hire.
• Current valid Massachusetts driver's license and driving record meeting BHCHP requirements.

Compensation and Benefits:

• The compensation increases based on years of experience and ranges from $42.60 - $68.15 hourly.
• BHCHP full-time employees are eligible for our competitive time off program, health, dental and vision insurance, 403B retirement savings plan, pre-tax MBTA pass program with 40% discount, additional compensation for demonstrated bilingual proficiency and more. Benefits are prorated for part-time employees.
