Registered Nurse – Care Manager (Inpatient RN)

Location: Bronx, NY

Type: Contract | 14 Weeks

Pay:
Local Rate: $2,350 – $2,500 Weekly
Travel Rate: $2,800 – $3,000 Weekly

Shift & Schedule: Monday–Friday | Day Shift | 40 Hours Weekly

We are seeking an experienced Registered Nurse Care Manager to support inpatient care coordination, utilization review, discharge planning, and patient transition services within an acute care hospital setting. The RN Care Manager will work closely with interdisciplinary teams to ensure appropriate resource utilization, quality patient outcomes, and safe discharge planning.

• Coordinate inpatient care management activities throughout the continuum of care.

• Perform utilization review to ensure appropriate level of care and resource utilization.

• Facilitate discharge planning and patient transition services.

• Collaborate with physicians, nursing staff, social workers, and ancillary departments to develop effective care plans.

• Identify barriers to discharge and implement appropriate interventions.

• Monitor patient progress and coordinate community-based resources when necessary.

• Participate in interdisciplinary rounds and care planning meetings.

• Maintain accurate documentation in accordance with hospital policies and regulatory standards.

• Support patient advocacy and quality improvement initiatives.

• Utilize electronic medical records and care management systems effectively.

• Bachelor of Science in Nursing (BSN) required.

• Master's Degree preferred.

• Active Registered Nurse (RN) License.

• Minimum 5 years of clinical RN experience.

• Experience in Care Coordination, Utilization Review, and Discharge Planning.

• Strong computer literacy and documentation skills.

• Excellent communication and interdisciplinary collaboration abilities.

• Acute Care Hospital Case Management experience.

• Certified Case Manager (CCM).

• Experience with EPIC or other electronic medical record systems.
