Job Title: RN - Care Manager
Location: Bronx, NY
Type: Contract
Shift & Schedule: Monday – Friday | 9:00 AM – 5:00 PM
Pay: $50/hr - $65/hr (Based on Experience)

Position Overview:

Navitas Healthcare, LLC is seeking a RN - Care Manager for an exciting Contract job in Bronx, NY.

Responsibilities

• Coordinate patient care throughout the inpatient stay.

• Conduct utilization review and support appropriate level-of-care decisions.

• Develop and implement comprehensive discharge and transition-of-care plans.

• Collaborate with physicians, nursing staff, social workers, and external agencies.

• Assess patient needs and ensure timely interventions and safe discharge planning.

• Maintain accurate and timely documentation of care plans and patient outcomes.

• Ensure compliance with hospital policies, regulatory guidelines, and payer requirements.

Required Qualifications

• Bachelor of Science in Nursing (BSN).

• Active New York State Registered Nurse (RN) License.

• Minimum 5 years of clinical RN experience.

• Prior experience in Care Coordination, Utilization Review, and Discharge Planning.

• PRI & Screen Certification (required or willingness to obtain if applicable).

• Current Basic Life Support (BLS) Certification.

• Strong proficiency with EMR systems and computer applications.

• Excellent communication, critical thinking, and organizational skills.

Preferred Qualifications

• Experience working in an acute care hospital setting.

• Strong interdisciplinary care coordination experience.

• Knowledge of payer guidelines and utilization management processes.

• Experience with discharge planning and transition-of-care programs.
