As a Physical Therapist, you will:

• Provide professional, evidence-based care in accordance with physician orders and agency policies.
• Help clients regain independence and improve quality of life through personalized rehabilitation plans.

Key responsibilities include:

• Conduct comprehensive assessments and develop individualized treatment plans.
• Deliver therapeutic interventions (manual therapy, modalities, exercise programs).
• Educate clients, families, and caregivers on therapy techniques and home programs.
• Supervise PTAs and Home Health Aides (as permitted).
• Collaborate with interdisciplinary teams and participate in care conferences.
• Monitor progress and adjust treatment plans as needed.
• Maintain accurate, timely documentation.

About us:

MercyOne Des Moines Home Care and Hospice is part of the MercyOne health system and a faith-based, not-for-profit agency. We provide skilled nursing, therapy, and medical social work services across Iowa.

Qualifications:

• Degree from an APTA-accredited Physical Therapy program
• Active state PT license
• 1+ year of experience or completion of a 68 week preceptorship
• Home health experience preferred
• Valid driver's license and reliable transportation
• Strong organizational and time-management skills

Physical & Mental Requirements:

• Ability to lift up to 35 lbs and assist with mobility
• Frequent standing, walking, bending, and positioning
• Adaptability to changing schedules and environments
• Travel to client homes and Trinity Health locations required
