Gentiva Hospice is seeking a dedicated Hospice RN/Case Manager to lead patients and families through the hospice journey in their home or preferred setting. You will deliver hands-on nursing care, assess patient needs, and collaborate with an interdisciplinary team to refine personalized care plans.
