RN/LPN Wellness Nurse

Overview Maplewood Brewster LLC, a senior living community specializing in assisted living and memory care in Brewster, MA, is seeking a compassionate and clinically skilled RN/LPN Wellness Nurse. In this role, you will play a central part in supporting residents’ health, independence, and overall quality of life within a warm, engaging, and resident-centered environment. Responsibilities Provide direct, hands-on nursing care to residents, including ongoing health assessments, medication administration, treatments, and coordination of individualized wellness plans. Monitor changes in residents’ conditions, respond promptly to clinical needs, and communicate effectively with physicians, families, and the interdisciplinary care team to ensure tailored, clinically appropriate care. Document care and observations in electronic medical records; follow infection control and safety protocols; participate in care plan meetings; and support caregivers with clinical guidance. Work closely with residents living with memory impairment using patience, empathy, and specialized approaches to support dignity, comfort, and emotional well-being. Help create an environment that minimizes behavioral distress and maximizes engagement, safety, and meaningful daily experiences. Collaborate with care managers, activities staff, and other team members to align clinical care with residents’ preferences, functional abilities, and life histories. Qualifications and Expectations Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a compassionate, relationship-based approach and the ability to thrive in a dynamic senior care setting. Comfort prioritizing multiple clinical needs, providing clear direction to support staff, and educating residents and families about care plans, medications, and wellness strategies. Ability to collaborate with physicians, families, and the interprofessional team to align care with residents’ preferences and life histories. Strong communication, documentation, and clinical judgment skills; commitment to infection control and safe care practices.
