RN Field Case Manager
LiveWell Homecare Agency, LLC
Seeking a registered nurse to provide in-home assessments, patient education, and care coordination in a community-based setting. Responsibilities include evaluating members’ health, identifying treatment barriers, making referrals, and supporting self-management. Maintain communication with members through in-person visits or phone interactions to ensure quality care continuity.
