Somatus is hiring a Field Based Nurse Case Manager in the greater Seattle area. You will outreaching and scheduling member visits in homes, dialysis clinics, and physician offices to support higher risk members needing barrier assessments and care plan education. In this role you will travel to home and facility visits, participate in care planning, educate on conditions, review medications, and connect to community resources while building trusted relationships with nephrology practices to #J-18808-Ljbffr Somatus
