Our full-service program supports adults, children and adolescents, and includes inpatient services, a multidisciplinary outpatient clinic, intensive outpatient treatment, emergency and consultative services and behavioral medicine services embedded in primary care.This position provides clinical services (behavioral health, substance abuse, health behavior change) aligned with the Primary Care Behavioral Health (PCBH) model. Applicants should have an interest in and/or experience with brief interventions, team-based care, a wide variety of psychiatric, substance abuse and behavioral medicine problems, and diverse populations.ResponsibilitiesThe Behavioral Health Clinician, as part of an integrated care team, identifies, triages, and manages patients with behavioral health problems within a variety of settings. In addition, this individual will provide skill training through patient education strategies and will develop specific behavioral health change plans for individual patients and behavioral health protocols for target populations.Performs counseling and case management for each patient assigned, including: assessment; diagnosis; planning care; intervention (including counseling patients/significant others, facilitating brief psychotherapy and educational groups, planning discharges); evaluation (including identifying unmet or new therapy services).Demonstrates the therapeutic core conditions of genuineness, empathy and positive regard when interacting with patients and significant others, according to generally accepted clinical practice.Performs risk management documentation and follow-up action requirements, e.g. responding to patient complaints, contributing to completion of Problem Resolution Report and Report of Adverse Risk Event (R.A.R.E).Recommends program cost control measures to supervisor.Participates in the internal Continuous Quality Improvement (CQI) process, such as peer review, audits, Joint Commission survey preparation, and implementation of clinical paths.Selects and gives clinical supervision to Adjunctive Therapists and student interns to ensure these individuals practice appropriate behavior- and skill-based competencies for each of these positions.Keeps an accurate, complete and current status of documentation onto the patients chart.Recommends safety and therapeutic quality measures for unit areas or rooms to charge nurse.Conveys professional role model example for other staff through quality of own clinical practice; work habits; availing self to others as a consultant on clinical issues; attending ongoing internal and external professional development events.Performs other functions as required.Additional Job Description:Masters DegreeNM LCSW or LPCCWe offer more than the standard benefits!Presbyterian employees gain access to a robust wellness program, including free access to our on-site and community-based gyms, nutrition coaching and classes, wellness challenges and more!
