The Company provides patient-focused precision medical care to patients in assisted living facilities and is committed to delivering the highest quality of care.

The Position: Our client is seeking a Family Nurse Practitioner.

$140,000k+ W2 or 1099 option available

Requirements:

Master's degree in Nursing and graduate of an accredited Nurse Practitioner program

Unrestricted New York State Nurse Practitioner license

Unrestricted DEA license to prescribe all commonly prescribed medications

Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid

Responsibilities:

Monday Friday (8am 5pm)

No Nights

No Weekends

Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation

Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident

Manage chronic conditions and acute illnesses, providing timely interventions and appropriate referrals as needed
