RN Registered Nurse

Job Overview BAYADA Home Health Care is hiring a Registered Nurse (RN) for Home Health visits in Revere, Massachusetts. The role involves delivering skilled nursing and rehabilitative care to patients in their homes while working collaboratively with a multi-disciplinary healthcare team. Responsibilities Perform comprehensive head‑to‑toe assessments to identify subtle changes in patient condition. Utilize expertise in acute and post‑acute care (e.g., CHF, COPD, wound care, IV therapy, post‑surgical care). Triage and manage urgent clinical situations independently in the home setting. Perform medication reconciliation, teach patients and caregivers, and monitor safety. Conduct thorough wound assessment, provide wound care, and maintain accurate documentation. Make real‑time clinical decisions and know when to seek escalation. Manage a dynamic visit schedule, prioritize urgent visits, and maintain productivity while ensuring quality care. Use point‑of‑care documentation tools efficiently and maintain attention to detail. Deliver care in varied home environments with cultural sensitivity and respect for patient autonomy. Qualifications Current and good‑standing RN license in the state of Massachusetts. Graduation from an accredited nursing program (transcript or diploma). At least one year of recent hands‑on medical experience (within the past two years) with adult or geriatric patients in acute care, emergency department, home health, hospice, ICU, med‑surg, sub‑acute care, or rehab settings. Experience with OASIS documentation. Ability to work independently, prioritize, and adapt to unpredictable home environments.
