We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team in Washington Township, OH. Kauhale Centerville is a memory care community. The LPN will provide high-quality nursing care to patients, assist with treatment plans, and support the healthcare team in delivering excellent patient outcomes. Key Responsibilities Provide direct resident care under the supervision of Director of Health and Wellness. Administer medications and treatments as prescribed by healthcare providers. Monitor and record patients’ vital signs and health status. Assist with patient assessments and report changes in condition to the healthcare team. Maintain accurate and detailed patient records and documentation. Educate patients and their families on health maintenance and disease prevention. Ensure compliance with healthcare regulations and facility policies. Collaborate with interdisciplinary team members to coordinate patient care. Qualifications Current and valid LPN license in the state of Ohio. Completion of an accredited practical nursing program. Previous experience in a clinical or healthcare setting preferred. Strong communication and interpersonal skills. Ability to work effectively both independently and as part of a team. Basic computer skills and familiarity with electronic health records (EHR) systems. CPR and Basic Life Support (BLS) certification preferred. This position is based in Washington Township, OH, providing care in a fast-paced healthcare environment. The role may require standing for extended periods, lifting or moving patients, and working various shifts including weekends and holidays.
