Provides personal care to agency patients. Records patient information in accordance with agency standards, policies, and procedures. Provides emotional support to patients and families, as needed.

Duties and Responsibilities:
1. Provide personal care, including baths, oral hygiene, shampoos, and changing bed linens according to the HHA plan of care (Patient care information in the plan of care).
2. Meets safety needs of patients and uses equipment safely and appropriately. This includes maintaining a clean and orderly patient care area. Uses proper body mechanics.
3. Assists in dressing and undressing patients.
4. Assists patients at meal times. This includes, but is not limited to, feeding, meal preparation as defined in the HHA plan of care/Daily Care information in the Plan of Care.
5. Provide proper care and observation of the patient's skin to prevent breakdown of tissue over bony prominences.
6. Report on patient's condition and significant changes.
7. Assist in ambulation and exercise as directed in the HHA plan of care. (Daily care information in the Plan of Care).
8. Offer and assist with bedpans and urinals.
9. Assist with personal laundry, light housekeeping related to patient care area.
10. Accurately and thoroughly document care provided on appropriate agency forms. Meet agency standards for clinical documentation.
11. Holds in strictest confidence all patient information and discloses information and data only to persons authorized by the agency.
12. Responsible for learning and utilizing technology (as an example but not limited to: phones, cell phones, computers) made available by the agency to increase efficiency, communication, and performance of their duties.
13. Follows agency policies and procedures.
14. Participates in continuing education programs.
15. Other duties as assigned by supervisor.

Minimum Qualifications:
1. Has completed a home health aide training program or an equivalent training program that meets the specific requirements of the state in which the aide will work. MUST HAVE A CERTIFICATE TO PROVE THIS.
2. Has at least 1 year of work experience in a home health agency, hospital, or long-term care facility.
3. At least 18 years of age.
4. Is a high school graduate or equivalent.
5. Can read, write, and carry out instructions.
6. Has physical ability necessary for extensive standing, lifting, and pushing.
7. Demonstrates tact, patience, and positive interpersonal skills necessary to work productively with patients, families, and co-workers.
8. Is flexible and cooperative in fulfilling role obligations.
9. Demonstrates good personal hygiene and will dress in clean, presentable uniforms.
10. Comes to work on time.

• Thursday, 8am - 3pm (first shift) or 3pm - 9pm (second shift),
• Friday, 8am - 3pm (first shift) or 3pm - 12am (second shift),
• Saturday, 9am - 5pm (first shift) or 5pm - 12am (second shift),
• Sunday, 9am - 3pm (first shift) or 3pm - 9pm (second shift) or 10am - 8pm (every other Sunday)
