Home Care Transition NurseMake a lasting impact by supporting patients through a safe and successful transition home. The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals at St. Joesph's hospital in Bethpage.The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals.5 x 8 hour shifts (9:30AM to 5:30PM) - Rotating weekends and holidaysGreat healthcare benefits, and education reimbursement of $5,250 annuallyAdditional compensation for visits over your caseloadJob DetailsOur Home Care Transition Nurses:Conduct patient assessments and develop individualized transition plansCoordinate referrals and facilitate smooth transitions to home care servicesBuild and maintain strong relationships with physicians, discharge planners, and referral sourcesServe as a key ambassador for Catholic Home Care within the communityIdentify opportunities for program growth and service expansionDeliver exceptional customer service and support to patients, families, and healthcare partnersCollaborate with interdisciplinary teams to ensure positive patient outcomesRequirements:Current Registered Nurse (RN) license in New York StateMinimum of 2 years of home care experience, including patient assessmentsStrong relationship-building, communication, and customer service skillsExperience in healthcare marketing, referral development, or business growth is a plusPassion for patient advocacy and care coordinationPosted Salary RangeUSD $95,000.00 - USD $120,000.00 /Yr.This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience.
