At SIRS, our Wellness Coordinators play a vital role in supporting individuals with intellectual and developmental disabilities by coordinating healthcare services, promoting wellness, and helping each person achieve their best possible quality of life. What You'll Do

Develop individualized Wellness Coordination and Risk Plans.
Coordinate healthcare services and collaborate with physicians, providers, and interdisciplinary teams.
Conduct in-home wellness visits and health assessments.
Monitor health outcomes and promote preventive care.
Train and support Direct Support Professionals on individual health and risk plans.
Oversee medication administration competency and compliance.
Maintain accurate documentation and ensure compliance with Indiana state guidelines.
Participate in Individualized Support Team meetings and advocate for the health and wellness of the individuals you serve.

What You'll Bring

Current Indiana Registered Nurse (RN) license.
CPR/First Aid certification (required before your first active day of employment).
Excellent communication, organization, and time management skills.
Reliable transportation, valid driver's license, and current automobile insurance.
Experience supporting individuals with intellectual and developmental disabilities is preferred but not required.
