Day shift, Monday through Friday, 8:00 AM - 5:00 PM Job Description We are seeking a skilled Registered Nurse (RN) to join our Home Health team. The RN will be responsible for managing patient care within the home setting, ensuring the delivery of high-quality nursing services. This position involves conducting assessments, planning care, and coordinating services for adult and geriatric patients discharged from hospitals, medical offices, and skilled nursing facilities. Responsibilities Conduct patient assessments and develop individualized care plans. Provide medication administration and management. Perform clinical assessments and documentation via HomeCare HomeBase (HCHB). Coordinate and manage patient services ensuring compliance with care plans. Educate patients and families on health conditions and care plans. Monitor and document changes in patient conditions. Required Experience / Certifications / Licensure Active RN license in the state of Maryland. Minimum of 1 year of home health nursing experience required. Experience using HomeCare HomeBase (HCHB) is required. BLS certification is required.
