Case Manager

The Case Manager plays a key role in engaging and supporting youth through program enrollment, individualized coaching, regular check-ins, activity planning, workshop facilitation, advocacy, and referrals to community resources. Programming focuses on academic exploration, career planning, life skills, professional development, and community engagement. Working closely with the Program Manager and other program staff, the Case Manager will help ensure youth have access to essential resources and services while creating opportunities to build meaningful connections, strengthen socioemotional skills, and develop the tools needed for lifelong learning and success. This position requires a strong commitment to youth development and the ability to inspire and empower young people.

This full-time position is 40 hours per week, with four days on-site and one day of virtual work available after a successful 60-day performance review.

Latin American Youth Center's (LAYC) mission is to empower youth to achieve a successful transition to adulthood, through comprehensive and innovative programs that address youths' social, academic, and career needs. ESSENTIAL RESPONSIBILITIES

Key Responsibilities:

Youth Engagement & Case Management:

• Conduct assessments for new youth and develop an IDP. Update IDP at least every 90 days with service planning on short and long-term goals.
• Manage a caseload of youth participants, providing regular check-ins to identify barriers to success and create space to develop goals and career planning.
• Build meaningful relationships with youth using Positive Youth Development (PYD) principles to understand and support their strengths, interests, goals, and needs.
• Make appropriate internal and external referrals and follow up to ensure youth receive required services in a timely manner.
• Coordinate with service providers, case managers, and other partners to support continuity of services and ensure youth have access to available resources.

Data Management & Documentation:

• Prepare, organize, and maintain accurate case files for each participant. Regularly enter and update participant engagement and service data in the Efforts to Outcome (ETO) database.
• Ensure participant documentation is complete, organized, and maintained in accordance with program and organizational requirements.
• Maintain accurate records and provide required data and reports to the Program Manager in a timely manner.

Department Collaboration (applies to all staff):

• Develop and facilitate activities and workshops in collaboration with the Program Manager.
• Collaborate with colleagues across programs to support major department events and youth-focused activities.
• Develop positive relationships and maintain regular communication with youth, colleagues, supervisors, and program partners. Ensure communication with colleagues happens regularly to ensure youth receive coordinated and comprehensive support.
• Work in partnership with Education, Workforce Development, case management, and other program staff to ensure youth are aware of and have access to available programming and services.
• Coordinate services across departments to support youth in achieving their education, employment, and personal development goals.
• Contribute to a collaborative, team-oriented environment by supporting organizational events, initiatives, and activities beyond primary program responsibilities.

Professional Development:

• Participate in agency trainings and professional development opportunities to strengthen skills and maintain alignment with LAYC's mission, Positive Youth Development approach, and program practices.
• Meet regularly with the Program Manager to review caseloads, participant progress, program goals, and outcomes.
• Seek opportunities to strengthen professional knowledge and effectiveness in supporting youth participants.

Compliance:

• Adhere to all LAYC personnel policies and complete all duties as assigned by the Program Manager.
• Maintain confidentiality and ensure participant information, case files, and program documentation are handled in accordance with organizational policies and requirements.
• Carry out additional responsibilities as assigned by the Program Manager.

EDUCATION & EXPERIENCE REQUIREMENTS

• A bachelor's degree in social work, counseling, psychology, or related field preferred
• Minimum of 1-2 years of experience working with inner-city adolescents and families, preferably in community-based and multicultural setting
• Experience working with out of school/at-risk youth preferred

SKILLS & QUALIFICATIONS

• Knowledge of community and local resources in Montgomery County
• Excellent oral and written skills
• Proficiency with Microsoft products: Word, Excel, PowerPoint, and Outlook
• Strong organizational skills and ability to work with minimum supervision
• Experience working in a team environment
• Ability to evaluate youth's emotional, behavioral, and social needs and determine the optimal

treatment strategies

• Complete medical, criminal, and child abuse and neglect clearances
• Flexible, energetic, positive individual with commitment to youth development in a high quality, respectful environment
• Ability to work well and communicate with culturally diverse populations
• Flexible schedule, including occasional evenings
• LAYC/MMYC requires the COVID-19 vaccine of all its employees. You will be asked to provide proof of vaccination at the time of your interview and at your onboarding.

At-Will Employment: This position is at-will, meaning that either you or LAYC may terminate the employment relationship at any time for any reason.

Performance Evaluation: In addition to an annual performance evaluation, there will be a 90-day performance review to ensure organizational fit and performance expectations are met.

PHYSICAL REQUIREMENTS

This is largely a sedentary position that requires the ability to speak, hear, see, and lift small objects up to 10 lbs. May require the ability to travel locally and/or regionally.
