Position Summary The Hospice Aide provides personal care, assistance with performing activities of daily living, prescribed therapeutic exercises, and basic housekeeping services essential to the comfort and cleanliness of recipients of home care or hospice services. These services are specified by a written plan of care developed and supervised by a Registered Nurse. Responsibilities Provides direct patient/basic nursing care and simple housekeeping tasks as reflected on the Home Health Aide Plan of Care under the direction and supervision of a Registered Nurse or other licensed professional. Patient care duties include bathing, pericare (with or without indwelling catheter), oral care, skin, hair and fingernail care; helping patients to move, turn, get in and out of bed, chair, or wheelchair, and ambulate with or without assistive devices. Provides care of surgical or prosthetic appliances or devices as instructed (e.g., catheters, colostomies, leg braces, dentures). Collects and processes specimens (e.g., urine dips, chart results) and reports to medical staff; obtains vital signs (blood pressure, heart rate, temperature, O2 saturation, weights) as assigned; measures and calculates fluid and food intake and output; performs simple dressing changes; assists with routine bowel care. Performs simple household duties that facilitate patient self‑care and prevent or postpone institutionalization. Provides in‑home hospice respite care when assigned. Assists with prescribed exercises taught by Physical, Speech, Occupational Therapists or Registered Nurse, demonstrating knowledge of range‑of‑motion, body alignment, and positioning techniques. Observes and reports changes in participant's physical or mental status to immediate supervisor; records such observations; communicates safety hazards to primary clinician and team members. Demonstrates effective time management, arriving in a timely manner, prioritizing and triaging visits, and communicating schedule with Clinical Manager, RN Case Manager, or Scheduler. Independently manages own caseload, plans and schedules home visits, and coordinates with other team members to maximize care plan effectiveness. Organizes weekly schedule to attend required meetings within patient care needs. Coordinates transfer of patient information for coverage during absences. Maintains accurate and appropriate records and documentation of status and services; documents deviations from plan of care; contacts primary clinician or Home Health Manager/Supervisor. Uses laptop computer to document patient care and communicate with team members, when needed. May train family members in safe patient hygiene practices. Promotes the Patient Bill of Rights and Responsibilities and understands grievance procedures to address participant concerns. Participates in relevant educational activities and attends a minimum of 12 hours of in‑service education per calendar year to meet state, federal, and agency requirements. Provides care to hospice patients as indicated by the Plan of Care, shares weekend and evening coverage, and provides immediate care to a single hospice patient during increased need. Education and Experience Required: Completion of a certified nursing assistant course or equivalent training. Required: One year of previous home health/hospice experience as a nurse’s aide, hospice aide, orderly, or related position in an acute, rehabilitative, or long‑term care facility, hospice organization, or private home care aide. Skills Mathematical: Ability to add, subtract, multiply, and divide in all units of measure; compute rate, ratio, and percentage. Language: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations; ability to write reports, business correspondence, and procedure manuals; strong written and verbal communication. Ability to communicate effectively in English, both verbally and in writing. Certifications, Licenses, and Registrations Required: Nursing Assistant Certification (Vendor Managed). Required within 30 days of hire: National Provider BLS – American Heart Association (Vendor Managed). HHA Certification required. Physical Demands and Work Environment The demands of this role necessitate regular standing, walking, and manual dexterity, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
