Description

• Conduct assessments for patients in either inpatient or outpatient environments and formulate tailored treatment plans
• Provide physical therapy interventions concentrating on mobility, strength, balance, and functionality
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Educate patients and their families regarding therapeutic objectives, discharge procedures, and home exercise regimens
• Document evaluations, treatments, and progress following facility and regulatory protocols
• Engage in interdisciplinary conferences and care team round discussions to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (such as neurological or geriatric) are advantageous but not required
