Established in 2018, Vitae has grown to support over 300 facilities across the Midwest and East Coast, focusing on integrated care that addresses both medical and behavioral health needs.We are seeking a compassionate and skilled Psychiatric Nurse Practitioner to join our interdisciplinary team. The ideal candidate will provide high-quality psychiatric evaluation, diagnosis, and management of mental health conditions for residents in skilled nursing facilities, collaborating closely with physicians, therapists, and other healthcare professionals to enhance patient well-being and quality of life.Key ResponsibilitiesConduct comprehensive psychiatric assessments and evaluations of residents, diagnosing mental health disorders and developing individualized treatment plans.Prescribe and manage psychotropic medications, monitoring effectiveness and side effects while adjusting therapies as needed.Collaborate with interdisciplinary teams including physicians, psychologists, social workers, and nursing staff to coordinate holistic patient care.Provide psychotherapy and counseling services where applicable to support residents' mental health and coping strategies.Participate in care plan meetings, quality improvement initiatives, and value-based care programs aimed at enhancing psychiatric outcomes and reducing hospital readmissions.Educate residents, families, and facility staff on mental health conditions, treatment plans, and behavioral interventions.Maintain accurate and timely documentation in compliance with clinical and regulatory standards.
