We are seeking a compassionate, organized, and self-motivated Social Worker / Geriatric Care Manager to join our growing team. This is an excellent opportunity for a social worker who is passionate about working with older adults, enjoys building meaningful relationships, working independently in the field, and collaborating with an experienced and supportive team.

What You'll Do

\n

As a member of our care management team, you will:

\n

\n
• Provide comprehensive care management and ongoing support to older adults living in the community
\n
• Conduct regular in-home visits and assessments throughout your assigned territory
\n
• Advocate for clients and help them navigate healthcare, housing, financial, and social service systems
\n
• Assist clients in accessing benefits, entitlements, and community resources
\n
• Develop individualized service plans and monitor client progress
\n
• Collaborate with family members, healthcare providers, and other members of the client's support network
\n
• Maintain timely and accurate case documentation
\n
• Participate in weekly team meetings and case conferences
\n
• Contribute to program development and continuous quality improvement initiatives
\n

Position Requirements

\n

Qualified candidates will possess:

\n

\n
• LMSW or LCSW
\n
• Experience working with older adults, care management, case management, or related human services fields
\n
• Knowledge of Medicare, Medicaid, Social Security, senior housing, and community-based services
\n
• Excellent interpersonal, communication, and advocacy skills
\n
• Strong organizational, documentation, and time-management abilities
\n
• Ability to work independently while remaining an active member of a collaborative team
\n
• Proficiency with Microsoft Office and electronic documentation systems
\n

Travel & Work Environment

\n

This position combines field-based client visits with administrative work and team collaboration.

\n

Candidates must:

\n

\n
• Be comfortable conducting regular home visits throughout the service area (NYC, Westchester, Long Island, New Jersey)
\n
• Have a valid driver's license and reliable transportation a plus
\n
• Be willing to travel locally to meet with clients, families, and community partners
\n
• Attend weekly in-person team meetings at our office
\n
• Maintain flexibility to adjust schedules based on client needs
\n
