Heart Failure Cardiologist

Position Overview Heart Failure Cardiologist at Cleveland Clinic Fairview Hospital, part of Cleveland Clinic Regional Cardiovascular Medicine. The role involves creating a heart failure program within the hospital, focusing on hospital care systems, population health management, and identification of patients for advanced treatments. Responsibilities Create systems of care for high quality and efficient hospital care of heart failure patients. Population health management of outpatients with heart failure. Appropriate identification and follow-up of patients needing advanced procedural treatments. Multidisciplinary coordination of care with primary care, cardiovascular specialists, and procedural teams. Electronic medical record population health analytics. Coordinate care closely with Cleveland Clinic Main Campus for mechanical support options and cardiac transplantation, and round on Main Campus advanced heart failure services as needed. Qualifications Board Certified/Board Examiner in Cardiovascular Medicine. Advanced subspecialty training in heart failure strongly encouraged. Faculty appointment at rank commensurate with experience available at Cleveland Clinic Lerner College of Medicine of Case Western Reserve University. Compensation and Benefits Salary range: $375,000 - $606,250. The pay range reflects the anticipated range for new hires and is for full‑time employment, adjusted for experience, board certification and other factors.
