Position Registered Nurse – Home Health Client Location: Tempe, AZ Company Overview We are a confidential home health company that provides high-quality, personalized care to our clients. Our team is dedicated to helping individuals maintain their independence and improve their overall health and well-being. Position Overview As a Registered Nurse for our home health client, you will provide skilled nursing care to patients in their homes. You will work closely with the interdisciplinary team to develop and implement individualized care plans, monitor patient progress, and provide education and support to patients and their families. This is an onsite position based in Tempe, AZ. Key Responsibilities Conduct initial assessments and ongoing evaluations of patient health status Develop and implement individualized care plans in collaboration with the interdisciplinary team Administer medications, treatments, and procedures as prescribed by the physician Monitor patient progress and make necessary adjustments to care plans Provide education and support to patients and their families on disease management, medication management, and other aspects of care Document all patient care activities accurately and timely Communicate with physicians and other healthcare professionals to coordinate care and ensure quality outcomes Maintain a safe and clean environment for patients and their families Adhere to all company policies, procedures, and regulations Qualifications Current and unrestricted Registered Nurse license in the state of Arizona Minimum of 1 year of nursing experience, preferably in home health or a related field Strong clinical skills and knowledge of home health regulations and standards of practice Excellent communication and interpersonal skills Ability to work independently and as part of a team Compassionate and patient‑centered approach to care Must be able to travel to patients' homes within the Tempe, AZ area We Offer Competitive salary and benefits package Opportunities for professional growth and development Supportive and collaborative work environment Flexible schedule options Meaningful work that makes a difference in the lives of our clients and their families #J-18808-Ljbffr Fortune Careers Limited
