As a Registered Nurse (RN) Case Manager - Hospice, you will have the opportunity to make a meaningful difference in the lives of our patients and their loved ones. Responsibilities Perform initial and ongoing assessments of patients and their families to develop individualized plans of care Coordinate care with the interdisciplinary team, including physicians, social workers, and other healthcare professionals Provide ongoing patient and family education and support to ensure optimal care and quality of life Identify and address potential barriers to care, such as psychosocial and spiritual concerns, and develop strategies to overcome them Collaborate with the hospice team to ensure seamless care transitions and effective communication with all involved parties Maintain accurate and timely documentation of patient care and communication with the healthcare team Qualifications Current licensure as a Registered Nurse (RN) in the state of Pennsylvania Strong clinical knowledge and skills, with a focus on hospice and palliative care Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals Strong organizational and prioritization skills, with the ability to manage multiple responsibilities and deadlines Ability to work in a fast-paced environment and adapt to changing situations and priorities About Serenity Hospice Care Pennsylvania Serenity Hospice Care Pennsylvania is dedicated to providing quality and compassionate end-of-life care and supportive services for patients and their families.
