Perinatal Health Nurse

Responsibilities Conduct home visitation services for medically diagnosed high‑risk pregnant, post‑partum and inter‑conceptual women. Provide nurse case management, refer for support, conduct education, and connect to community resources. Conduct outreach to reduce disparities in birth outcomes. Provide general clinical support within the home. Communicate with health care providers regarding client status updates throughout the program. Maintain a valid driver’s license. Qualifications PH Nurse 1: Associate degree in nursing from an accredited college or university, completion of a PHN course within one year of hire, current Georgia Registered Professional Nurse license, and CPR certification. PH Nurse 2: Associate degree in nursing from an accredited college or university, one year of nursing experience, completion of a PHN course within one year of hire, current Georgia Registered Professional Nurse license, and CPR certification; or one year of experience at the lower level PH Registered Nurse 1 (HCP190) or equivalent. PH Nurse 3: Associate degree in nursing from an accredited college or university, one year of nursing experience, completion of a PHN course within one year of hire, current Georgia Registered Professional Nurse license, and CPR certification; or two years of experience at the lower level PH Registered Nurse 3 (HCP192) or equivalent. Preferred: BSN, prior experience in home care or medical case management, newborn or pediatric experience, bilingual in Spanish, one year of Labor & Delivery experience or one year working in a prenatal care clinic, or one year experience working with mothers and babies.
