Provides home visiting services within the 14-county health district to identify, assess, and provide care coordination services to eligible perinatal patients and qualifying infants.
Identifies high-risk populations and tailors interventions based on the needs of the population and resources available.
Provides outstanding customer service and maintains HIPAA standards related to Privacy and Security.
Under broad supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures. Promotes the program through community engagement and outreach events.
Provides referrals and linkages to appropriate services when medical and environmental risks are identified.
Responds to Public Health Emergencies
Associate degree/diploma in Nursing from an accredited college AND Current Georgia Registered Professional Nurse license. (Bachelor's OR Master's degree in nursing, public health, or related field from an accredited college or university
Previous employment with any state's Department of Public Health or a County Board of Health
Experience with providing intervention services for high-risk newborns and infants
Experience with Case Management and Community Outreach
One or more years of Labor and Delivery experience
Bilingual (Spanish and English).
